Please note that this form is only for new grievances. If you have already filed a grievance, do not use this form to send us additional information. Mail any additional information with your grievance file number to our office address or send it to the email address intake@wsba.org.
You will receive a confirmation email after you submit your grievance. We will communicate with you by letter after we review your grievance.
